Throughout the year release forms for most upcoming events requiring them can be

General Program
Registration Form
This general program registration form must be completed before regular
participation in any of our youth ministry programming.
General Program Behavioral Contract
Completed acknowledgement page signed by both youth and parent(s) must accompany
all program registrations. Signing indicates that youth and parent(s) have
received, read, understand, and agree to comply with the General Program
Behavioral Contract.
Youth
Health and Medical Form
For safety and liability reasons, all program registrations must be accompanied
by a completed Youth Health and Medical Form.
Service
Verification Form
Verification form for Christian Service Hours for Confirmation program.
Adult Volunteer Registration Form
All adults who will have regular supervisory contact with minors must fully
complete and submit this form and then go through a registration process
including an interview, safe environment training, and fingerprinting process to
volunteer with St. Bernardine Youth Ministry.
Adult
Health and Medical Form
For safety and liability reasons, all adult volunteers must complete an Adult
Health and Medical Form.
Driver
Information Sheet
Before volunteering to drive for youth ministry events, all drivers must
complete this form. The Archdiocese of
Los Angeles does not permit
drivers under the age of 25 to transport minors.
For safe environment
reasons, all drivers transporting minors for youth ministry events must be
accompanied by second adult driver age 25 or older.
